All on 4

Full-Arch Restoration

A full arch of teeth. Four implants.

All-on-4 replaces an entire arch of missing or failing teeth with a fixed, non-removable set of teeth supported by four strategically placed implants. Dr. Andrey Ilyabayev plans and places All-on-4 restorations for patients across Bayside, Queens, and the surrounding NY area.

What Is All-on-4?

All-on-4 replaces an entire row of missing or failing teeth using just four dental implants per arch, two placed straight in the front of the jaw and two angled toward the back. That angled placement takes advantage of the denser bone typically found at the front of the jaw, which often means patients can avoid a separate bone grafting procedure. The four implants support a fixed, custom-designed arch of teeth that does not come out, unlike a traditional denture.

Why Patients Choose All-on-4

Fixed, Not Removable
Anchored to four implants, All-on-4 does not come out and functions like a permanent set of teeth.
Preserves Jawbone
The implants stimulate the jawbone the way natural roots do, helping prevent further bone loss.
Often Avoids Bone Grafting
The angled posterior implants take advantage of denser front-of-jaw bone, often eliminating the need for a separate graft.
Same-Day Temporary Teeth
A temporary fixed arch is often attached the same day as surgery, so you are not without teeth during healing.

All-on-4 Compared to Traditional Dentures

Patients considering a full-arch restoration are usually weighing All-on-4 against a traditional removable denture. Looking at the two side by side makes the practical differences clear.

Traditional Denture All-on-4
Rests on the gums, which accelerates jawbone loss over time Anchored to implants, which helps preserve jawbone density
Held in place with adhesive, which needs daily reapplication Fixed in place, no adhesive required
Can shift or slip while eating or speaking Does not move, functions like natural teeth
Some foods are difficult or impossible to eat Most foods can be eaten normally once healed
Removed nightly for cleaning Brushed in place like natural teeth
Often needs relining or replacement every 5 to 10 years as the jawbone changes shape Designed as a long-term, fixed solution

Who Is a Candidate for All-on-4?

All-on-4 is designed for patients who are already missing most or all of their teeth in an arch, or whose remaining teeth are failing due to decay, gum disease, or injury and are not realistically savable. Dr. Ilyabayev evaluates your bone density, gum health, and dental history before recommending this or any full-arch option, so the plan matches what your mouth actually needs rather than a one-size answer.

Extensive Tooth Loss
Missing most or all teeth in an arch
Failing Teeth
Remaining teeth compromised by decay or disease
Ill-Fitting Dentures
Frustrated with slipping or unstable dentures
Reduced Bone Volume
May still qualify without a separate bone graft

The All-on-4 Process

01 / PLAN

Consultation & 3D Imaging
Dr. Ilyabayev maps your jawbone with 3D CBCT imaging to plan exact implant position and angle before any surgery begins.
02 / EXTRACT & PLACE

Extractions & Implant Placement
Any remaining unhealthy teeth are removed at the start of the same visit, then four implants are placed per arch, two straight and two angled.
03 / PROVISIONAL

Same-Day Temporary Arch
A temporary fixed arch, designed in advance from your scans, is attached the same day, so you are not without teeth during healing.
04 / HEAL

Osseointegration
Over roughly four to six months, the bone fuses to the implants. Many patients return to work or normal activity within a day or two of surgery.
05 / FINAL

Final Restoration
Once healing is confirmed, your in-house lab-crafted permanent arch is fitted at a follow-up visit.

Frequently Asked Questions

How Long Does All-on-4 Last?

All-on-4 is designed as a long-term, fixed solution. A published follow-up case study tracked an All-on-4 fixed prosthesis for 8.5 years with successful long-term function, though outcomes depend on bone health and consistent maintenance (NIH/PMC case report). Regular checkups at Bayside Dental Arts are the biggest factor in long-term success.

Will I Be Without Teeth During Treatment?

No. A temporary, fixed arch is typically attached the same day your implants are placed, so you leave with a functional set of teeth while the permanent restoration is designed.

How Long Is Recovery After All-on-4?

Many patients return to work or their normal routine within a day or two of surgery, though some swelling and soreness in the first few days is normal. Full healing and osseointegration, where the bone fuses to the implants, typically takes four to six months before the final restoration is placed.

What Is the Difference Between All-on-4 and All-on-6?

All-on-6 uses six implants per arch instead of four, offering additional support for patients who need it. Dr. Ilyabayev will recommend whichever option matches your bone structure and treatment goals.

Does Insurance Cover All-on-4?

Coverage varies by plan. Because All-on-4 is a larger treatment plan, Bayside Dental Arts offers a free consultation to walk through your options, along with PPO insurance and financing through CareCredit, Cherry, Alphaeon, and LendingClub.
